Among many political, economic, and cultural factors influencing social capital, the spatial dimension is most often overlooked. Yet the accessibility of places promoting social interactions and interpersonal communication between local residents should have a direct effect on the vitality of a neighborhood. Among these civic places are formal institutions - churches and locally oriented membership organizations, and informal, or “third places” such as small retail establishments including coffee shops, pubs, markets, and beauty parlors. While having different functions they all provide opportunities for individuals, and especially - for local residents, to transcend the “work-home” dichotomy, to become a part of local social networks. We assess the vitality of urban neighborhoods in the city of Portland, Oregon, using four indicators: residential stability, degree of racial integration, and the frequency of property crimes and crimes against persons. We hypothesize that the spatial concentration of civic places in neighborhoods should be associated with a higher share of non-migrants, higher racial integration, and lower per capita crime rates. These neighborhood outcomes serve as indicators of social capital and have received little or no attention in earlier research. This analysis is conducted at the census block group level using a geographic information system (GIS) to generate spatial indices of urban accessibility, and an ordinary least squares model to examine the spatial distribution of civic organizations on neighborhood vitality. This research contributes in several ways to the theoretical and methodological debates related to the concept of social capital. The results suggest that civic places influence neighborhood vitality outcomes and provides additional evidence that such places may facilitate formation of social capital by creating opportunities for informal interactions and civic engagement. We argue that the formal definition of social capital should be expanded to include its spatial component - the notion of place - together with social and psychological components of networks, norms, and social trust. The increasing human impact on the underlying natural processes and ecosystems upon which these wide-ranging benefits depend, represents the single greatest threat to the sustainability of fisheries. A large proportion of the world’s fisheries are currently over-exploited, and as a result are underperforming in terms of their full biological and economic potential. Ineffective fisheries management continues to perpetuate these negative outcomes. To-date, a majority of investments in fisheries management improvements have disproportionately depended on public and philanthropic sources of funding, with few cases of private-sector investment being deployed despite the potential upside to fisheries recovery. The latter is at least in part the result of a lack of project pipeline for fisheries investment. The OPP project responds directly to this challenge by providing technical assistance and analytical support to identify and design a series of fisheries investment proposals that are attractive to return-seeking investors. The latter will in-turn catalyze pilot investment into selected transformational public-private partnerships that mainstream the sustainable management of highly migratory fish stocks spanning areas within and beyond national jurisdictions. They are five times more likely to die from diabetes. The goal of this study was to examine how articles published in leading medical journals between 1989 and 1998 accounted for such differences in health outcomes among Black and white women. The explanatory content of the articles was analyzed and coded according to four types of attributions: genetic/biological, cultural/behavioral, structural/socioeconomic and alternative. Each type of explanation derives from different assumptions and operates with different models for understanding why health outcomes vary among groups. Alternative explanations are those that focus on the direct effect of race/gender oppression on Black women's health. Genetic/biological attributions occurred less frequently than structural/socioeconomic and cultural behavioral but were more likely to occur than alternative attributions, which were the least often employed. While alternative attributions are considered in some of the articles about Black women's health and mortality, they are overall rarely employed. The finding that explanations that most directly explore the impact of racism and sexism on Black women's health occur least often has important implications. We conducted a retrospective cohort study from October, 2011 through October, 2014, using statewide PDMP data, hospitalization registry, and vital records.Early PDMP registrants (n=927) were matched with clinicians who never registered during the study period, using baseline prescribing metrics in a propensity score. Generalized estimating equations were used to examine prescribing trends following PDMP registration, using 2-month intervals. We found a statewide decline in measures of per capita opioid prescribing. However, compared with non-registrants, PDMP registrants did not subsequently have significantly fewer patients receiving high-dose prescriptions; overlapping opioid and benzodiazepine prescriptions, inappropriate prescriptions, prescriptions from multiple prescribers, or overdose events. At baseline, frequent PDMP users wrote fewer high-risk opioid prescriptions than infrequent users; this persisted during follow-up with few significant group differences in trend. Thus, although opioid prescribing declined statewide after implementing the PDMP, registrants did not demonstrate greater declines than non-registrants. The purpose of this research was to examine the relationships between psychosocial stress, social support, institutionalized constraints, and their impacts on health and well-being among adolescent mothers in the Northwestern United States. Guided by a biocultural approach and mixed methods design, this study involved five primary research activities and sources of data: 1) long-term, extensive participant-observation in teen parent and doula communities; 2) semi-structured, open-ended interviews with two comparative cohorts--doula-supported adolescent mothers (n=24) and non-doula supported adolescent mothers (n=18); 3) semi-structured, open-ended interviews with doulas that provide care to young mothers (n=15); 4) descriptive analyses of maternal-infant health outcomes from a national sample of doula-supported adolescent women (n=1892); and 5) content analysis of published literature on doula professional standards, core competencies, scopes of practice, and care philosophies. Through a series of manuscripts, I examine doulas from four perspectives, that of: young mothers, doulas, outcomes-based health data, and textual representations of doula models of care. In using a multiple vantage point approach, I am able to situate understandings of doula care within the larger social contexts and structural forces that frame the experiences of young mothers and the integration of doulas into maternity reform strategies. In doing so, I explore how psychosocial stress, social support, and institutionalized constraints pattern the negotiations, aspirations, and outcomes of young women during their emergence into motherhood. By focusing on the synergy between micro- and macro-level elements, I demonstrate the potential for doula care, as a targeted form of social support, to advance health equity and longterm social well-being for young mothers and their families. The aim of this study was to examine the effect of military service with consideration for the demographic statuses of race, gender, and educational attainment on economic outcomes. Data for this study came from the Current Population Survey July 2010 with Veterans Supplement (N=83,000). Results from this study suggest that some veterans, namely those of minority racial status and lower educational attainment benefit from their military serve by achieving increased levels of household income as compared to similar non-veterans. Conversely, non-Hispanic White veterans and those with higher levels of educational attainment suffer negative consequences to levels of household income. Furthermore, differences among veterans were assessed. These results provide further insight into the experiences of veterans in the United States. Data for this study came from the Current Population Survey July 2010 with Veterans Supplement (N=83,000). Results from this study suggest that some veterans, namely those of minority racial status and lower educational attainment benefit from their military serve by achieving increased levels of household income as compared to similar non-veterans. Conversely, non-Hispanic White veterans and those with higher levels of educational attainment suffer negative consequences to levels of household income. Furthermore, differences among veterans were assessed. These results provide further insight into the experiences of veterans in the United States. Close

This research aims to study the application of a set of metrics within one perspective of the Balanced Scorecard (BSC) framework, called learning and growth, in healthcare organizations. Using data provided by managers from clinics and hospitals, the most important learning and growth performance categories that drive the future performance of a healthcare organization are determined. Moreover, to monitor these performance categories, a set of metrics are identified. Such capabilities will strengthen the ability of the governments of coastal developing countries to either exclude DWFVs from their exclusive economic zones (EEZs) or negotiate and enforce better agreements. Their unprecedented increase in conjunction with anthropogenically induced range shifts of endemic pathogens exposes hosts to novel parasite combinations, lending urgency to research on disease dynamics in wildlife systems. In natural populations, hosts must concurrently battle infection from multiple parasite species. Host fitness, disease severity, transmission rate, host susceptibility, and effective control all depend on the dynamics of the parasite community. However, the bulk of research on infectious diseases—their transmission and fitness effects—is limited to one-host and one-parasite systems. It has become apparent that a broader perspective, which incorporates interactions among pathogens within and between hosts, is necessary to predict the outcome of co-infection. In this dissertation, I examined the role of co-infection for host immunology, host fitness, and disease dynamics. Specifically, I examined the role of two intestinal parasites, coccidia and nematodes, and two intracellular pathogens, Brucella abortus and Mycobacterium tuberculosis, in a free-ranging population of African buffalo. First, I examined coccidia and nematode infection patterns in buffalo at Hluhluwe Imfolozi Park (HIP), South Africa, to test the hypothesis that co-infection and season interact to influence the distribution and severity of infections (Chapter 2). I tested for a role of infection and co-infection on three proxies for host fitness: host pregnancy, host body condition, and parasite aggregation. By examining host fitness costs across seasons and in different demographic groups, this work showed that the effects of infection were most pronounced in the dry season and with co-infection. Then, I investigated the infection patterns, host fitness, and population level consequences of B. abortus infection in African buffalo of Kruger National Park (KNP), South Africa. I evaluated the use of an enzyme-linked immunosorbent assay (ELISA, IDEXX Brucellosis Serum Ab Test) for diagnosis of brucellosis, the disease caused by B. abortus (Chapter 3), and applied it to understand the environmental and demographic variation in the individual-level survival and fecundity costs of infection (Chapter 4). Brucellosis infection was associated with increased mortality and condition-dependent reductions in reproductive success. By summarizing these individual level consequences with a mathematical model, I explored how condition- dependent variability in vital rate parameters and disease effects translates into contrasting outcomes of brucellosis infection for population growth. Finally, I re-examined the influence of co-infection in the context of brucellosis by investigating how an emerging pathogen, M. bovis, the causative agent of bovine tuberculosis (bTB), alters host immunity, infection patterns, fitness costs, and the dynamics of brucellosis infection (Chapter 5). The purpose of this study is to explore the dynamic linkages of personality and health by examining how daily health-goal progress is associated with daily positive and negative affect as well as whether the association between health-goal progress and positive and negative affect can be predicted by personality traits of neuroticism and conscientiousness. Main research questions are: 1) Are daily positive and negative affect associated with daily health-goal progress? 2) To what extent are the traits of neuroticism and conscientiousness related to the overall levels of daily health-goal progress over a 100-day time period? 3) Do neuroticism and conscientiousness moderate the relationships between daily positive affect and daily health-goal progress as well as between negative affect and daily health-goal progress? This study was guided by developmental systems theory (Ford & Lerner, 1992) and the six-foci model of personality (Hooker & McAdams, 2003). This study utilized data from the "Personal Understanding of Life and Social Experiences" (PULSE) project that was a 100 day internet-based, daily study of Oregon residents over the age of 50. The sample for this study (N = 76) included participants who have participated at both baseline (initial survey) and the microlongitudinal phase (over 100 days). Personal health goals, neuroticism, and conscientiousness were measured at baseline. Daily health-goal progress, daily positive affect, and negative affect were measured over 100 days. Multilevel modeling analysis was used to examine within-person variations and between-person differences in daily health-goal progress and daily positive and negative affect by estimating an intercept (initial status) and slope (change) for each individual. The results of this study show that first, daily health-goal progress was positively coupled with daily positive affect and negatively coupled with daily negative affect within persons. Second, the associations between daily positive affect and daily health-goal progress and between daily negative affect and daily health-goal progress varied between individuals. Third, health-goal progress on the previous day was positively related to concurrent positive affect and negatively related to concurrent negative affect. Fourth, individuals high in neuroticism and individuals high in conscientiousness were only marginally likely to experience higher levels of health-goal progress over the 100-day period compared to those with low scores. Fifth, individuals high in neuroticism when experiencing high levels of negative affect tended to report lower levels of daily health-goal progress. These findings may provide enhanced knowledge of patterns of day-to-day variability within persons and lead to better health care. Moreover, the findings of the current study suggest that health-improving interventions could be targeted individually to participants based on knowledge of the linkages between daily goal progress and daily affect and personality traits.
