Methodology/Principal Findings: To understand the level, distribution, and correlates of ART adherence behavior, we prospectively determined monthly ART adherence through multiple measures and six-monthly HIV RNA levels among 121 Ugandan children aged 2–10 years for one year. Median adherence levels were 100% by three-day recall, 97.4% by 30-day visual analog scale, 97.3% by unannounced pill count/liquid formulation weights, and 96.3% by medication event monitors (MEMS). Interruptions in MEMS adherence of $48 hours were seen in 57.0% of children; 36.3% had detectable HIV RNA at one year. Only MEMS correlated significantly with HIV RNA levels (r = 20.25, p = 0.04). Multivariable regression found the following to be associated with ,90% MEMS adherence: hospitalization of child (adjusted odds ratio [AOR] 3.0, 95% confidence interval [CI] 1.6–5.5; p = 0.001), liquid formulation use (AOR 1.4, 95%CI 1.0–2.0; p = 0.04), and caregiver’s alcohol use (AOR 3.1, 95%CI 1.8–5.2; p,0.0001). Child’s use of co-trimoxazole (AOR 0.5, 95%CI 0.4–0.9; p = 0.009), caregiver’s use of ART (AOR 0.6, 95%CI 0.4–0.9; p = 0.03), possible caregiver depression (AOR 0.6, 95%CI 0.4–0.8; p = 0.001), and caregiver feeling ashamed of child’s HIV status (AOR 0.5, 95%CI 0.3–0.6; p,0.0001) were protective against ,90% MEMS adherence. Change in drug manufacturer (AOR 4.1, 95%CI 1.5–11.5; p = 0.009) and caregiver’s alcohol use (AOR 5.5, 95%CI 2.8–10.7; p,0.0001) were associated with $48-hour interruptions by MEMS, while second-line ART (AOR 0.3, 95%CI 0.1–0.99; p = 0.049) and increasing assets (AOR 0.7, 95%CI 0.6–0.9; p = 0.0007) were protective against these interruptions. Conclusions/Significance: Adherence success depends on a well-established medication taking routine, including caregiver support and adequate education on medication changes. Caregiver-reported depression and shame may reflect fear of poor outcomes, functioning as motivation for the child to adhere. Further research is needed to better understand and build on these key influential factors for adherence intervention development. Safety was similar among all groups. Median MEMS adherence rates were 83% [IQR: 63–92] for daily dosing and 55% [IQR:28–78] for fixed intermittent dosing (p = 0.003), while adherence to any post-coital doses was 26% [IQR:14–50]. SMS response rates were low, which may have impaired measurement of post-coital dosing adherence. Acceptability of PrEP was high, regardless of dosing regimen. Conclusions/Significance: Adherence to intermittent dosing regimens, fixed doses, and in particular coitally-dependent doses, may be more difficult than adherence to daily dosing. However, intermittent dosing may still be appropriate for PrEP if intracellular drug levels, which correlate with prevention of HIV acquisition, can be attained with less than daily dosing and if barriers to adherence can be addressed. Additional drug level data, qualitative data on adherence barriers, and better methods to measure sexual activity are necessary to determine whether adherence to post-coital PrEP could be comparable to more standard regimens. Methods/design: The COMPASS (COMmunity of Practice And Safety Support) study is a cluster randomized trial being implemented among Oregon’s unionized home care workers. Partnering with the Oregon Home Care Commission allowed recruiting 10 pairs of home care worker groups with 8 participants per group (n = 160) for balanced randomization of groups to intervention and control conditions. Physiologic and survey evaluation of all participants will be at enrollment, 6 months and 12 months. Primary outcomes are to increase health promoting (for example, healthy nutrition and regular physical activity) and health protecting (that is, safety) behaviors. In addition to assessing outcomes adjusted for the hierarchical design, mediation analyses will be used to deconstruct and confirm the program’s theoretical underpinnings and intervention processes. Intervention groups will participate in a series of monthly 2-hour meetings designed as ritualized, scripted peer-led sessions to increase knowledge, practice skills and build support for healthy actions. Self-monitoring and individual and team level goals are included to augment change. Because generalizability, reach and achieving dissemination are priorities, following initial wave findings, a second wave of COMPASS groups will be recruited and enrolled with tailoring of the program to align with existing Home Care Commission educational offerings. Outcomes, process and mediation of those tailored groups will be compared with the original wave’s findings. Discussion: The COMPASS trial will assess a novel program to enhance the safety and health of a vulnerable, rapidly expanding group of isolated caregivers, whose critical work allows independent living of frail seniors and the disabled. This commerce plays a vital role in sustaining the regional economy and connecting Oregon to the global economy. The timely connection of truck and rail transport with vessels is vital, especially for export traffic. This link is susceptible to disruption if water depths in the navigation channel are shallower than expected, leading to delays and/or draft limitations. Moreover, ship drafts have increased in recent decades, 25% of the vessels calling in the river sail with a draft close to the channel depth at low water, and these carry roughly 70% of the cargo. A large vessel may have as little as 0.6 m bed clearance when it passes through a low-tide point in the river, which each vessel in transit must do. Thus, prediction and real-time communication of water level to vessels is vital to safety as well as efficiency. This has been implemented through the LOADMAX system, consisting of telemetered water-level gauges and a forecast model. Moreover, the dilemma has been made more critical by changes in the river – low water levels in the river channel between Wauna and Vancouver have decreased 0.3-1.2 m since 1940. The rate of decrease depends on location and riverflow, but appears to have accelerated in the last decade. The reasons for this decrease are not understood. For perspective, an ongoing 0.9 m channel deepening will cost about $150 million when completed, so unintended decreases in water depths are expensive as well as potentially dangerous. Lower water levels in the river also increase carbon emissions, because smaller loads mean more land and vessel transport trips. Further, navigation and salmon habitat restoration are closely connected. Dredging is used to maintain the channel, and habitat restoration is an integral part of the channel deepening. New dredging strategies are needed to maintain the newly deepened channel, but dredging that removes material permanently from the river may, further lower water levels, limiting possibilities for habitat restoration and reducing bed clearance for large ships. These problems will be exacerbated by future decreases in summer river flows due to climate change. An implication of this multiscale homogenization hypothesis is that the ecosystem structure and function and human behaviors associated with urbanization should be more similar in certain kinds of urbanized locations across biogeophysical gradients than across urbanization gradients in places with similar biogeophysical characteristics. This paper introduces an analytical framework for testing this hypothesis, and applies the framework to the case of residential lawn care. This set of land management behaviors are often assumed-not demonstrated-to exhibit homogeneity. Multivariate analyses are conducted on telephone survey responses from a geographically stratified random sample of homeowners (n = 9,480), equally distributed across six US metropolitan areas. Two behaviors are examined: lawn fertilizing and irrigating. Limited support for strong homogenization is found at two scales (i.e., multi- and single-city; 2 of 36 cases), but significant support is found for homogenization at only one scale (22 cases) or at neither scale (12 cases). These results suggest that US lawn care behaviors are more differentiated in practice than in theory. Thus, even if the biophysical outcomes of urbanization are homogenizing, managing the associated sustainability implications may require a multiscale, differentiated approach because the underlying social practices appear relatively varied. Objective: We estimated risk differences (RD) of PTB (reported per 106 pregnancies) associated with change in ambient concentrations of elemental carbon (EC), organic carbon (OC), nitrates (NO3), and sulfates (SO4). Methods: From live birth certificates from three states, we constructed a cohort of singleton pregnancies at or beyond 20 weeks of gestation from 2000 through 2005 (n = 1,771,225; 8% PTB). We estimated mean species exposures for each week of gestation from monitor-corrected Community Multi-Scale Air Quality modeling data. RDs and 95% confidence intervals (CIs) for four PTB categories were estimated for each exposure using linear regression, adjusted for maternal race/ethnicity, marital status, education, age, smoking, maximum temperature, ozone, and season of conception. In this spirit, the Portland State University Center for Transportation Studies has been working with regional and statewide partners to develop new capabilities to measure, monitor and track the performance of the transportation system in real time and using archived data sources. We believe that it is possible to leverage these disparate data sources toward providing better transportation system performance information for planners, engineers, citizens, researchers and decision-makers. Using this information, we can collaboratively develop policies and programs that can help make our transportation system more efficient, equitable and effective. With this in mind, we are pleased to present the 2007 Portland Metropolitan Region Transportation System Performance Report. We have attempted to make this report comprehensive and multimodal in spirit. We truly view this as a starting point, a work in progress, and we intend to continue to improve the content and format of this report in years to come. Sequencing would also offer benefits for science investigations occurring on the surface of Mars by permitting identification of Earth-derived contamination in samples. If Mars contains indigenous life, and that life is based on nucleic acids or other closely related molecules, sequencing would serve as a critical tool for the characterization of those molecules. Therefore, spaceflight-compatible nucleic acid sequencing would be an important capability for both crew health and astrobiology exploration. Advances in sequencing technology on Earth have been driven largely by needs for higher throughput and read accuracy. Although some reduction in size has been achieved, nearly all commercially available sequencers are not compatible with spaceflight due to size, power, and operational requirements. We integrated information from a dynamic global vegetation model, a sage-grouse habitat climate envelope model, and a state-and-transition simulation model to project broad-scale vegetation dynamics and potential sage-grouse habitat across 23.5 million acres in southeastern Oregon. We evaluated four climate scenarios, including continuing current climate and three scenarios of global climate change, and three management scenarios, including no management, current management and a sage-grouse habitat restoration scenario. All climate change scenarios projected expansion of moist shrub steppe and contraction of dry shrub steppe, but climate scenarios varied widely in the projected extent of xeric shrub steppe, where hot, dry summer conditions are unfavorable for sage-grouse. Wildfire increased by 26% over the century under current climate due to exotic grass encroachment, and by two- to four-fold across all climate change scenarios as extreme fire years became more frequent. Exotic grasses rapidly expanded in all scenarios as large areas of the landscape initially in semi-degraded condition converted to exotic-dominated systems. Due to the combination of exotic grass invasion, juniper encroachment, and climatic unsuitability for sage-grouse, projected sage-grouse habitat declined in the first several decades, but increased in area under the three climate change scenarios later in the century, as moist shrub steppe increased and rangeland condition improved. Management activities in the model were generally unsuccessful in controlling exotic grass invasion but were effective in slowing woodland expansion.
