Whether for good or bad, the family profoundly affects the individual. The family is an important unit. Many stresses impinge upon it. The church and extended family are less influential on the family today. The school is less influential. Where does the family turn for support? Social agencies have become the source for low cost aid for the family. The following study explores the extent to which social agencies provide this support for families in this community. To that end, project investigators have developed a research model of work-family fit by means of an exhaustive literature review of over 350 articles and books on work and family, the secondary data analysis of the North Carolina Family Caregiver Survey (Brennan & Poertner, 1997), and a focus group study with 41 employed parents of children with emotional or behavioral disorders (Rosenzweig, Brennan, & Ogilvie, 1998). The work-family fit research model includes as antecedent conditions (a) types and characteristics of parental employment, (b) types and characteristics of the children’s educational and child care arrangements, (c) the social support available to family members, and (d) family household characteristics. The outcomes featured in the model encompass (a) parent stress levels, (b) employment absenteeism, (c) work interuptions and tardiness, and (d) difficulty combining work and family life. The model also contains such intervening variables as the strategies parents use to balance work and family demands and the services they use. Currently, pilot interviews are being conducted with 60 family members who are employed at least 30 hours per week and who have children living in the home between the ages of 6 and 18 years who have emotional or behavioral disorders. Year: 2016 Finding fit between work and family responsibilities is challenging for parents, especially when raising a child with mental health difficulties. The purpose of this study was to investigate the relationship of children’s problematic behaviors to maternal employment and difficulty combining work and family obligations. Data were analyzed from 174 mothers parenting a child with a diagnosis of attention deficit hyperactivity disorder who completed child problem behavior (child behavior checklist), family functioning, and employment measures at baseline and 12 months later. Results from logistic regression analysis indicated that children’s problematic behaviors predicted maternal employment. Structural equation modeling showed that children’s higher baseline scores on the child behavior checklist internalizing scale were related to difficulty combining work and family at 12 months, even after controlling for difficulty of combining work and family at baseline. Children’s externalizing scores were not found to predict difficulty combining work and family responsibilities. Further analysis did not confirm mediation between children’s externalizing and internalizing behaviors, family conflict, and difficulty combining work and family. Given these results, service providers need to more comprehensively address the intersection of children’s mental health difficulties and work–family fit in order to better support caregivers as parents and employees. Year: 2016 Beginning in February 2016, the North Douglas County (NDC) communities of Drain, Elkton, and Yoncalla, Oregon engaged in a Community Health Needs Assessment (CHNA) planning process, focusing on children ages birth-to-8 and their families. The purpose of this process was to explore, understand, and integrate linkages between early learning and young child and family health; identify health-related resources currently available and accessible to NDC families; and prioritize health areas of interest to be explored during the CHNA. The North Douglas County CHNA builds on community partnerships already established in the region through initiatives that aim to coordinate and align early learning and K-12 systems. The work of Yoncalla Early Works (“YEW”, initiated in 2012 with funding from The Ford Family Foundation and Children’s Institute) and North Douglas P3 (“NDP3”, initiated in 2014 with funding from the Oregon Community Foundation) demonstrate community commitment to supporting young children and families. The theoretical base of this study was a social ecological model of interactive influences including individual parent, family, and neighborhood level factors to predict outcomes. Protective Factor Index (PFI) and Risk Factor Index (RFI) predictors were developed to explore potential multi-level protective factor buffering effects on key child development and parenting outcomes. Participants were first time mothers enrolled in a randomized controlled study of the Healthy Start/ Healthy Families Oregon (HS/HFO) home visitation program (treatment group) who completed a follow-up phone survey at the child’s 12 month birthday (n = 405). Families were offered HS/HFO services prenatally after meeting risk screening eligibility criteria on the New Baby Questionnaire (NBQ). Program mothers having received at least one home visit (n = 248) were included in the final analyses. Families had an average of 3.1 (SD = 1.2) NBQ risk factors at enrollment and 83% reported having trouble paying for basic needs. Families received an average of 16 home visits in the first 6 months of the program. Thirty-one percent of mothers were aged 19 or younger, 60% were White and Non-Hispanic, 31% were Hispanic, and 9% were another race/ethnicity. Hierarchical regression models with main effects (RFI, PFI, race) and an interaction term (RFI X PFI) were developed to predict eight outcomes. Interaction effects models were not significant. Five RFI main effects were significant: higher RFI scores were associated with greater likelihood of child welfare involvement, greater parenting stress, less favorable scores on child health and well-being, lower parent responsiveness and ii acceptance, and less supportive learning environments. One PFI main effect was significant: higher PFI scores predicted lower parenting stress. A trend level result showed higher PFI scores were associated with less child welfare involvement. Race was significant in two models: White/Non-Hispanic families were more likely to have a home visitor report child welfare involvement and had more frequent parent-child activities compared to other race/ethnicity families. Unpacking the results with separate single risk factor (12 items) and protective factor (10 items) regression models followed. Results showed parent’s prior family history of maltreatment and younger maternal age predicted child welfare involvement (home visitor report), while protection was seen for those with access to housing support. Social support and family functioning protectors were linked to lower parenting stress, while maternal depression showed the opposite finding. Better scores on a child health and well-being measure were seen with higher neighborhood cohesion and greater participation in HS/HFO; in contrast, neighborhood violence and frequent mobility were linked to worse scores. Developmentally supportive home environments were seen for families participating in additional parent support programs, in which the mother had greater knowledge of infant milestones and behavior, and if the family had access to housing supports. Unemployment proved to be associated with less enriched home environments. In summary, there was no support for the cumulative PFI in buffering risk for negative outcomes in this model. The RFI was also a more robust predictor of outcomes compared to the PFI in the main effects models. Overall, study findings provide some evidence for the utility of specific protective factors, as well as cumulative and specific single risk factors, for screening families for effectively targeting services and guiding the conceptual development of program and evaluation formats. Hierarchical regression models with main effects (RFI, PFI, race) and an interaction term (RFI X PFI) were developed to predict eight outcomes. Interaction effects models were not significant. Five RFI main effects were significant: higher RFI scores were associated with greater likelihood of child welfare involvement, greater parenting stress, less favorable scores on child health and well-being, lower parent responsiveness and ii acceptance, and less supportive learning environments. One PFI main effect was significant: higher PFI scores predicted lower parenting stress. A trend level result showed higher PFI scores were associated with less child welfare involvement. Race was significant in two models: White/Non-Hispanic families were more likely to have a home visitor report child welfare involvement and had more frequent parent-child activities compared to other race/ethnicity families. Unpacking the results with separate single risk factor (12 items) and protective factor (10 items) regression models followed. Results showed parent’s prior family history of maltreatment and younger maternal age predicted child welfare involvement (home visitor report), while protection was seen for those with access to housing support. Social support and family functioning protectors were linked to lower parenting stress, while maternal depression showed the opposite finding. Better scores on a child health and well-being measure were seen with higher neighborhood cohesion and greater participation in HS/HFO; in contrast, neighborhood violence and frequent mobility were linked to worse scores. Developmentally supportive home environments were seen for families participating in additional parent support programs, in which the mother had greater knowledge of infant milestones and behavior, and if the family had access to housing supports. Unemployment proved to be associated with less enriched home environments. In summary, there was no support for the cumulative PFI in buffering risk for negative outcomes in this model. The RFI was also a more robust predictor of outcomes compared to the PFI in the main effects models. Overall, study findings provide some evidence for the utility of specific protective factors, as well as cumulative and specific single risk factors, for screening families for effectively targeting services and guiding the conceptual development of program and evaluation formats. The research questions investigated are the concordance of families and professionals in assessment of strengths, differences in assessment of strengths, problems in specific domains of functioning, and relationships of characteristics of the child with recognition of strengths by the parent and professional. This study uses data collected from families of children with serious emotional disorders receiving services through community-based wraparound services supported by the mental health services program for children #5 HSS SM52297 funded by the Center for Mental Health Services, Substance Abuse and Mental Health Administration. The dissertation research presents a secondary analysis of a portion of the evaluation data collected for that demonstration. Eighty-five children were assessed by the parent and professional using the Behavioral and Emotional Rating Scale (BERS), the Achenbach Child Behavior Checklist (CBCL), and the Child and Adolescent Functioning Scale (CAFAS). It was found that families provided significant and unique information regarding their children. This study was designed to achieve several specific objectives: (1) to conduct a large-scale randomized study of the impact of the Healthy Families Oregon (HFO) program, a home visitation program using the Healthy Families America model, on child welfare system involvement, access to self-sufficiency resources, and use of preventive and other medical services; (2) to conduct a comprehensive and detailed cost-benefit study of the HFO program; and (3) to develop and disseminate a web-based tool to support home visiting program managers, funders, and others to better understand and develop readiness for conducting program cost analysis and cost-benefit research. Approach. 2,727 eligible first-time parents were randomly assigned to receive the HFO program or a community services-as-usual control group. The research team obtained 2 years of postenrollment follow-up data on all study participants from Oregon’s child welfare system, selfsufficiency services, and medical assistance programs. Additionally, a 1-year post-enrollment telephone interview was conducted with a random sample of 803 study participants (403 program, 400 control). Analyses were also conducted to examine whether program impacts varied for subgroups of families with different demographic and baseline risk characteristics, and to explore the relationship of program fidelity and dosage to outcomes. Finally, a detailed program cost analysis was conducted and administrative data outcomes were used in a cost-benefit analysis. Results/Benefits. 1-year follow-up interviews with parents found that HFO families were significantly more likely (compared to controls) to read to their young children frequently, to provide developmentally supportive activities, and to report lower parenting-related stress as measured by the Parent Stress Inventory (PSI). These effects appeared to be strongest for families with four or more risk factors (two factors more than the threshold for “at risk”). Administrative data outcomes at 2 years post enrollment found that families were no more likely to have a founded child abuse report than were controls (6.3% vs. 6.0%), but were significantly more likely to have an unfounded report (9.7% vs. 7.9%). HFO families, compared to controls, were also significantly more likely to have been enrolled in TANF services for the first time, received more days of Supplemental Nutrition Assistance Program (SNAP), and were more likely to be enrolled in substance abuse treatment services. There were no significant differences between groups in terms of use of or access to publicly funded health insurance or health-related services. Consistent with other cost-benefit analyses of home visiting and early childhood programs, results of the cost-benefit analyses did not support short-term cost savings associated with receipt of the HFO program. This study was designed to achieve several specific objectives: (1) to conduct a large-scale randomized study of the impact of the Healthy Families Oregon (HFO) program, a home visitation program using the Healthy Families America model, on child welfare system involvement, access to self-sufficiency resources, and use of preventive and other medical services; (2) to conduct a comprehensive and detailed cost-benefit study of the HFO program; and (3) to develop and disseminate a web-based tool to support home visiting program managers, funders, and others to better understand and develop readiness for conducting program cost analysis and cost-benefit research. Approach. 2,727 eligible first-time parents were randomly assigned to receive the HFO program or a community services-as-usual control group. The research team obtained 2 years of postenrollment follow-up data on all study participants from Oregon’s child welfare system, selfsufficiency services, and medical assistance programs. Additionally, a 1-year post-enrollment telephone interview was conducted with a random sample of 803 study participants (403 program, 400 control). Analyses were also conducted to examine whether program impacts varied for subgroups of families with different demographic and baseline risk characteristics, and to explore the relationship of program fidelity and dosage to outcomes. Finally, a detailed program cost analysis was conducted and administrative data outcomes were used in a cost-benefit analysis. Results/Benefits. 1-year follow-up interviews with parents found that HFO families were significantly more likely (compared to controls) to read to their young children frequently, to provide developmentally supportive activities, and to report lower parenting-related stress as measured by the Parent Stress Inventory (PSI). These effects appeared to be strongest for families with four or more risk factors (two factors more than the threshold for “at risk”). Administrative data outcomes at 2 years post enrollment found that families were no more likely to have a founded child abuse report than were controls (6.3% vs. 6.0%), but were significantly more likely to have an unfounded report (9.7% vs. 7.9%). HFO families, compared to controls, were also significantly more likely to have been enrolled in TANF services for the first time, received more days of Supplemental Nutrition Assistance Program (SNAP), and were more likely to be enrolled in substance abuse treatment services. There were no significant differences between groups in terms of use of or access to publicly funded health insurance or health-related services. Consistent with other cost-benefit analyses of home visiting and early childhood programs, results of the cost-benefit analyses did not support short-term cost savings associated with receipt of the HFO program. They also may not disclose their children's special needs to their employer, unless difficulties arranging and keeping child care make disclosure necessary. Major policy issue but unknown to public because many families do not disclose. Reality is that 1 in 5 families have a child with special needs. Research implications---parents develop strategies to keep their jobs and keep child care. Parents often have to choose between keeping their jobs or accessing services because health care and mental health services are often available only during working hours. Research findings are that children are not accepted for child care due to their special needs; and if a provider does enroll them, parents are called often at work to handle child care problems, disrupting their work. Considering expulsion from child care: 1/4 parents asked to remove their child for behavioral issues vs. 1/10 for physical issues. Some families try to handle child care by tag-team parenting, and attempt to rearrange their work schedules so that they can care for the child at home. Nearly 20% of US children have emotional or behavioral problems which provide unique challenges to access services, keep up with regular school, or make transportation arrangements. Work-family integration problems are particularly acute for parents having children with emotional or behavioral challenges; 48% of parents of children with emotional or behavioral problems had to quit work at some time to care for their children at home. Children with behavioral needs are 20% more likely to be expelled from child care, than children without those challenges. The ideal situation for families that have children with special needs may be inclusive child care arrangements. They also may not disclose their children's special needs to their employer, unless difficulties arranging and keeping child care make disclosure necessary. Major policy issue but unknown to public because many families do not disclose. Reality is that 1 in 5 families have a child with special needs. Research implications---parents develop strategies to keep their jobs and keep child care. Parents often have to choose between keeping their jobs or accessing services because health care and mental health services are often available only during working hours. Research findings are that children are not accepted for child care due to their special needs; and if a provider does enroll them, parents are called often at work to handle child care problems, disrupting their work. Considering expulsion from child care: 1/4 parents asked to remove their child for behavioral issues vs. 1/10 for physical issues. Some families try to handle child care by tag-team parenting, and attempt to rearrange their work schedules so that they can care for the child at home. Nearly 20% of US children have emotional or behavioral problems which provide unique challenges to access services, keep up with regular school, or make transportation arrangements. Work-family integration problems are particularly acute for parents having children with emotional or behavioral challenges; 48% of parents of children with emotional or behavioral problems had to quit work at some time to care for their children at home. Children with behavioral needs are 20% more likely to be expelled from child care, than children without those challenges. The ideal situation for families that have children with special needs may be inclusive child care arrangements.
