In order to improve services to these families, there has been an increasing focus on improving collaboration between child welfare, treatment providers, and the court systems. This paper presents the results from qualitative interviews with 104 representatives of these three systems that explore how the collaborative process works to benefit families, as well as the barriers and supports for building successful collaborations. Results indicate that collaboration has at least three major functions: building shared value systems, improving communication, and providing a "team" of support. Each of these leads to different kinds of benefits for families as well as providers and has different implications for building successful collaborative interventions. Despite these putative benefits, providers within each system, however, continue to struggle to build effective collaborations, and they face such issues as deeply ingrained mistrust and continued lack of understanding of other systems' values, goals, and perspectives. As will be demonstrated below, very little is actually known about the process of divorce or about its impact on the members of the family. Of particular concern is the effect of divorce on the children invo1ved, and the decisions and practice of the divorcing parents regarding their subsequent parenting relationship to the children. The authors of this report intend to study a random sample of divorcing couples and to derive a descriptive analysis of their perceptions of the proper role of the noncustodial parent (usually the father) in regard to the child rearing process following separation and divorce. We will work with that portion of available data for which we have information from both parents in the divorcing family. Particular attention will be paid to issues of congruence or noncongruence in perceptions between the two members of the coparental couple (including each member's judgement of his/ her partner's perceptions of the issues. We believe that the ability of the divorcing parents to provide a stable and cooperative relationship in their parental roles is very important for the children to be assisted in adjusting to the divorce. Given this assumption the extent of congruence or noncongruence in the perceptions of the parents about the role of the noncustodial parent in postdivorce child rearing may be very important in determining the degree of cooperation between them and the subsequent ease of adjustment of the children to the new family role relationships. Our data will be taken from questionnaires and oral interviews with a random sample of divorcing couples, administered very soon after the official filing for the divorce. Limited studies rooted in Oregon exist to assist practitioners, policy makers, and scholars in understanding food insecurity and access among recent immigrants and refugees. Using a multiple qualitative methods approach, this study has three major aims: 1) to understand the food access issues, particularly barriers and opportunities, among refugees and immigrants; 2) to investigate the challenges and opportunities of immigrants and refugees in accessing systems that are culturally responsive to their needs and; and 3) to address the gaps of service delivery targeted for communities of color and explore potential opportunities for partnership and creating culturally responsive approaches. To address the first two aims, data was drawn from a photovoice with recorded dialog among youth-parent pairs and a “talking circle” with five refugee families. Additionally, to address the latter two aims, data was drawn from a service providers’ focus group, of five service providers in the Portland area and one individual interview with a city government worker. Content analyses were done with the photos and transcripts of the family dialogs and “talking circle,” service providers’ focus group, and the individual interview. Findings indicate emerging themes on the utilization of cultural resources in the community, stress and strain among refugee families, structural or organizational barriers or challenges to being culturally responsive, and opportunities for school and community-based partnerships. Implications and recommendations for culturally responsive approaches and ways to better serve communities of color pertaining to food security and access initiatives are provided. They also bring to the U.S.A. cultural orientation that is collectivist, and shared experiences of persecution and deep distrust of the government. As is detailed in this report, these experiences have an impact on health and wellbeing, and simultaneously have a detrimental effect on the economic stature of the community. This Community Needs Assessment on the Health of Slavic Children (ages 0-5 years) was conducted to produce a “needs assessment” report outlining specific health concerns, needs and indicators of health disparities within the Slavic community, best practices from other Slavic communities, and recommendations for next steps to address disparities in a culturally competent change in policy or practices. The community opted to conduct two major data gathering processes: a maternal and child survey, and focus groups with mothers. We supplemented this information with a literature review to highlight both the larger social and economic issues that are well-tied to health (such as income, poverty, school engagement and English language skills), alongside studies that have identified health assets and challenges in both Slavic and immigrant communities. The core priority of the community are to establish a Slavic Health Center where providers will share their language, culture and history, and where health needs will be understood in their cultural context, and the distrust that typifies the relationships that many in the community have with formal health and social services will be eliminated. The need for this is strong, in evidence through the survey, the focus groups and in the literature. Specialized services are warranted to address the health issues tied to being newcomer communities, and in holding cultural identities and experiences that diverge significantly from that of mainstream (white) society. Additional priorities include rectifying the deep divide that exists on the issue of immunizations, with this being the highest priority health issue for mothers with young children. Second on the priority list is for parents to be well prepared for their children to arrive at school, and to help make this transition easier on both parents and children. Third is to address a wide range of issues tied to health promotion, including learning and responding to issues of child neglect and the conditions that will promote family stability. Fourth is oral health, and the importance of being able to see Slavic oral health providers. Across the spectrum is a need for information on services available, as well as making such services available, and making them culturally responsive. Taking action on two levels is key for the community: putting empowerment-based information into the hands of Slavic families, and catalyzing among service providers enhanced abilities to serve the Slavic community in respectful and culturally dignified ways. Health disparities faced by Slavic community members are inequitably measured, simply because their race/ethnicity is not separated out from “White” in the vast majority of research studies. Unfortunately, data cannot be separated by NWHF’s equity priorities like ethnicity overall, and cultural or linguistic background in particular. Culturally and linguistically specific services targeting this community’s needs are less likely than other disadvantaged communities of color such as African American and Native American, particularly due to inadequate gathering of data beyond the traditional racial and ethnic breakdown. Geography impacts this community’s health simply because of poverty-driven gentrification and the likelihood of Slavic families living in low-income diverse neighborhoods with greater risks of health disparities such as high obesity prevalence and chronic disease. Low-income neighborhoods where some of these families reside are less likely to have culturally and linguistically specific services targeting healthy choices and behaviors. Year: 2010 Objectives: This study examined the relationship between substance use disorder (SUD) and intimate partner violence screening (IPV) and management practices in the emergency department (ED). Methods: This was a retrospective cohort study of adult ED patients presenting to an urban, tertiary care teaching hospital over a 4-month period. An automated electronic data abstraction process identified consecutive patients and retrieved visit characteristics, including results of three violence screening questions, demographic data, triage acuity, time of visit, and ICD-9 diagnosis codes. Data on management were collected using a standardized abstraction tool by two reviewers masked to the study question. Multivariate logistic regression was used to determine predictors of screening and management. Results: In 10,071 visits, 6,563 violence screens were completed. IPV screening was documented in 33.5% of patients with alcohol-related diagnoses (95% CI = 27.7% to 39.3%, ?2 = 116.78, p < 0.001) and 53.3% of patients with drug-related diagnoses (95% CI = 44.3% to 62.3%, ?2 = 7.69, p = 0.006), compared to 66.1% of patients without these diagnoses (95% CI = 65.2% to 67.1%). In the multivariate analysis, alcohol (OR 0.30, 95% CI = 0.22 to 0.40) and drug use (OR 0.56, 95% CI = 0.38 to 0.83) were associated with decreased odds of screening. Of completed screens, 429 (6.5%) were positive, but violence was addressed further in only 55.7% of patients. Substance abuse did not appear to affect the odds of having positive screens addressed further by providers (OR 1.96, 95% CI = 0.39 to 10.14). Conclusions: This study found an association between SUD and decreased odds of IPV screening. Failure to screen for IPV in the setting of substance use may represent a missed opportunity to address a critical health issue, and be a barrier to successful intervention. We are interested in the adjustment of the child to the changes which occur during separation and divorce, and we maintain the attitude that the lives of children are affected in some way when a marriage or family unit dissolves. A generally accepted belief is that divorce produces negative consequences in the lives of children as shown in their behavior. However, recently a number of studies have suggested that divorce need not be a negative experience, at least to the degree most people feel it is. Factors that influence the effects on children include: the intensity of the conflict between the divorcing partners; the degree to which parents demonstrate concern for the children and help them prepare for the separation; the child’s attachment to the custodial parent; and the level of maturity and general personality characteristics of the child. We are interested in the adjustment of the child to the changes which occur during separation and divorce, and we maintain the attitude that the lives of children are affected in some way when a marriage or family unit dissolves.
