There are three major aims of the study. The first is to measure variances in morale states as experienced and reported by the respondents. The second aim is to determine how a given set of factors correlate with the self-reported morale of each of the respondents. We describe the experimental implementation of a collaborative, stepped-care model for the treatment of late-life depression (GermanIMPACT trial) in the German primary care context. GermanIMPACT was developed as an adaptation of a successful and widely used American model. The aim of the study is to evaluate the model’s applicability to the German primary care setting and its cost-effectiveness. Methods/Design: The study will be conducted as a cluster-randomized controlled trial comparing the development of depressive symptoms in primary care patients who either receive treatment as usual (control arm) or treatment according to the GermanIMPACT model (intervention arm). In two German cities (Freiburg and Hamburg), a total of 60 general practice offices will be selected and randomized. Each general practice office will be asked to enroll five patients into the trial who are 60 years of age or older and who show moderate depressive symptoms in the scope of a diagnosed depressive episode, recurrent depressive disorder, or dysthymia. General practices in the control arm will provide treatment as usual; general practices in the intervention arm will work closely with a specially trained care manager and a supervising mental health specialist. Evidence-based elements of the treatment plan manual include patient education, identification and integration of positive activities into the daily routine, relapse prevention, and training of problem-solving techniques as needed. The intervention period per patient will be one year. Data will be collected at baseline, 6, and 12 months. Primary outcome is the patient-reported change of depressive symptoms (Patient Health Questionnaire, PHQ-9). Secondary outcomes include measures of quality of life, anxiety, depression-related behavior, problem-solving skills, resilience, and an overall economic evaluation of the program. Discussion: The GermanIMPACT trial will provide evidence about the effectiveness, feasibility, and cost-effectiveness of collaborative stepped care in treating late-life depression in German primary care. Positive results will be a first step toward integrating specialized depression care managers into the primary care setting. Key changes between the 2008 OOHPR survey and 2014 include: Compared to 2008, the number of facilities increased by 13%, with the largest growth in MCCs (41%). The proportion of for-profit facilities and facilities managed by a third party increased from 2008 by 8%. The acuity level of residents increased on most measures compared to 2008. Compared to 2008, residents across all three community types required more assistance with ADLs and used more health services. Residents of MCCs required the most assistance with ADLs and use of health services, which is similar to 2008. The percent of residents who used hospice services increased from 2008 by 2%. The percent of residents who visited the hospital or an emergency department increased from 10% in 2008 to 28% in 2014. The percent of residents using Medicaid increased from 2008 by 10%. The purpose of the study was to determine: (1) the factors that influence driving cessation; (2) the physical and emotional barriers that delay driving cessation; (3) what opportunities exist for alternative transportation after driving cessation; (4) whether drivers make relocation decisions on the basis of driving cessation; (5) the warning signs that make a driver stop driving; and (6) whether a crisis situation generally forces a driver to stop driving. Completed mail surveys included those from 342 respondents who were current drivers (184 urban and 141 rural, plus 17 who did not report whether they lived in an urban or rural area) and 158 respondents who had voluntarily ceased driving (110 urban and 37 rural, plus 11 who did not report their urban/rural status). Telephone interviews were completed with 33 urban drivers, 36 rural drivers, 25 urban ceasers, and 6 rural ceasers. Changes in driving patterns, occurred gradually and late, mostly for respondents in their late 70’s or early 80’s. Among the results of the study was the finding that those most likely to have chosen to stop driving were older, depressed females in poorer health who were living in senior housing, using alternative transportation when available, making fewer trips, and seeing fewer limitations associated with using alternative transportation. Relocation to improve access to transportation alternatives was not seen by most respondents as a viable option. Because this study was a cross-sectional, not longitudinal, analysis, it was not possible to determine causality (e.g., to know whether people became depressed as a result of ceasing to drive or being depressed led them to cease driving, or to know whether poor health was the result or cause of driving cessation). Additional research is needed to establish causality, ideally following groups of individuals in particular age cohorts over time, including individuals forced to stop driving through the Medically-At-Risk Program. Such research would enable better understanding of the factors affecting driving decisions and the effects of those decisions, and help to signal possible strategies to implement to encourage the use of alternative transportation. We are interested in the adjustment of the child to the changes which occur during separation and divorce, and we maintain the attitude that the lives of children are affected in some way when a marriage or family unit dissolves. A generally accepted belief is that divorce produces negative consequences in the lives of children as shown in their behavior. However, recently a number of studies have suggested that divorce need not be a negative experience, at least to the degree most people feel it is. Factors that influence the effects on children include: the intensity of the conflict between the divorcing partners; the degree to which parents demonstrate concern for the children and help them prepare for the separation; the child’s attachment to the custodial parent; and the level of maturity and general personality characteristics of the child. The primary goal of the study presented here is to determine the extent to which negative behavior changes in children are visible immediately following a divorce. Additionally we wish to describe those changes and determine their frequency. Researchers, planners, policymakers, and community residents have been increasingly interested in shaping urban environments as places that foster active aging and independence among older adults. Senior centers have served a critical role in their communities as focal points for older adults, as individuals or in groups, to participate in services and activities that support their independence and encourage their involvement in and with the community (NCOA, 1979). The aging of the population and influx of baby boomers into the older demographic, along with declines in participation rates and funding, are challenging senior center staff to be innovative and adaptive in order to demonstrate senior centers’ relevance to future older adults while maintaining the programming and services for their current participants. This research is a multiple-case study of five unique senior centers located throughout the Portland metropolitan area in Oregon. Different types and models of senior centers are represented by the selected cases, as they operate under different governance structures and vary in aspects such as administration, funding, participant characteristics, community partnerships, transportation options, physical environments, and range of services and programs offered. This study aims to better understand challenges, strengths, and future directions for different types of senior centers serving a metropolitan area. Data collection at each site included interviews with staff as well as focus groups with senior center participants. Interviews with key informants enriched the findings and provided outside perspectives of senior center experiences. Data analysis revealed challenges and strengths that were common across and unique to the different senior centers in this study. This research contributes to the literature in urban studies, community development, and gerontology by exploring potential opportunities for urban senior centers to adapt and continue to serve older adults. Recommendations were developed for how senior centers might capitalize on these opportunities and for how their communities might provide mechanisms of support to facilitate the continuation and contributions of senior centers in metropolitan areas. This research contributes to the literature in urban studies, community development, and gerontology by exploring potential opportunities for urban senior centers to adapt and continue to serve older adults. Recommendations were developed for how senior centers might capitalize on these opportunities and for how their communities might provide mechanisms of support to facilitate the continuation and contributions of senior centers in metropolitan areas.
