  Citation × Citation Citation Abstract Copy Title: Come Out Come Out Wherever You Are: A Content Analysis of Homeless Transgender Youth in Social Service Literature Author: Crossley, Shannon Year: 2015 Homeless, transgender youth are underrepresented in research studies in the United States and over-represented among the lesbian, gay, bisexual, transgender and queer (LGBTQ) transitional-age (16 – 24) homeless youth population, at a reported 20 - 40% of the entire homeless youth population. Although this figure is staggering, it is not representative of the actual number of LGBTQ youth currently living on the streets. Exact numbers are unknown due to fear of victimization and discrimination faced both by their homeless peers and in services that are established for homeless youth. This study critically examined how homeless, transgender, transitional-age youth are represented in or ignored by the social service scholarly literature. The study implemented a thematic analysis of academic articles from 2008 - 2013 with a specific focus on homeless, transitional-age youth found in the Social Services Abstract database. Themes identified from the selected articles relate to perceptions of services, barriers to care, gender identity, risk factors, and a comparison of heterosexual to homosexual homeless youth. These factors will be used to determine the level of representation or exclusion from available studies about homeless transgender youth. The most common services were central storage for medications and cueing residents, while the least common were administering injections and intravenous medications. Medication reminders, helping residents take medications, and administering drops/topical ointments and injections varied by RCC characteristics. Characteristics most commonly associated with these differences are size, purposefully built status, nursing hours, and availability of a physician or pharmacist to review medication appropriateness. Understanding these MMS variations may benefit clinicians’ efforts to achieve medication adherence among RCC patients, and inform policy makers, RCC providers, and consumers. We describe the experimental implementation of a collaborative, stepped-care model for the treatment of late-life depression (GermanIMPACT trial) in the German primary care context. GermanIMPACT was developed as an adaptation of a successful and widely used American model. The aim of the study is to evaluate the model’s applicability to the German primary care setting and its cost-effectiveness. Methods/Design: The study will be conducted as a cluster-randomized controlled trial comparing the development of depressive symptoms in primary care patients who either receive treatment as usual (control arm) or treatment according to the GermanIMPACT model (intervention arm). In two German cities (Freiburg and Hamburg), a total of 60 general practice offices will be selected and randomized. Each general practice office will be asked to enroll five patients into the trial who are 60 years of age or older and who show moderate depressive symptoms in the scope of a diagnosed depressive episode, recurrent depressive disorder, or dysthymia. General practices in the control arm will provide treatment as usual; general practices in the intervention arm will work closely with a specially trained care manager and a supervising mental health specialist. Evidence-based elements of the treatment plan manual include patient education, identification and integration of positive activities into the daily routine, relapse prevention, and training of problem-solving techniques as needed. The intervention period per patient will be one year. Data will be collected at baseline, 6, and 12 months. Primary outcome is the patient-reported change of depressive symptoms (Patient Health Questionnaire, PHQ-9). Secondary outcomes include measures of quality of life, anxiety, depression-related behavior, problem-solving skills, resilience, and an overall economic evaluation of the program. Discussion: The GermanIMPACT trial will provide evidence about the effectiveness, feasibility, and cost-effectiveness of collaborative stepped care in treating late-life depression in German primary care. Positive results will be a first step toward integrating specialized depression care managers into the primary care setting. Key changes between the 2008 OOHPR survey and 2014 include: Compared to 2008, the number of facilities increased by 13%, with the largest growth in MCCs (41%). The proportion of for-profit facilities and facilities managed by a third party increased from 2008 by 8%. The acuity level of residents increased on most measures compared to 2008. Compared to 2008, residents across all three community types required more assistance with ADLs and used more health services. Residents of MCCs required the most assistance with ADLs and use of health services, which is similar to 2008. The percent of residents who used hospice services increased from 2008 by 2%. The percent of residents who visited the hospital or an emergency department increased from 10% in 2008 to 28% in 2014. The percent of residents using Medicaid increased from 2008 by 10%.
