Year: 1971 This is an exploratory follow-up study of the clientele of the Psychiatric Crisis Unit, a short-term, crisis-oriented inpatient psychiatric ward. The main objective of the research was to test the following null hypothesis: there are no significant differences between those individuals who attempt to gain aftercare treatment as opposed to those individuals who do not following discharge from the Crisis Unit. A sample of fifty-one voluntary patients who consented to participate in the study was used in testing this hypothesis. Each subject completed the Minnesota Multiphasic Personality Inventory (M.M.P.I.) and a sociological questionnaire while in the Crisis Unit, and a follow-up questionnaire was administered via telephone or personal contact approximately one month after discharge. The follow-up information was used to determine whether the subject fell into the "aftercare” or “no-aftercare” group. Data collection lasted from July 1, 1970 to December 15, 1970. The data revealed that there were significant differences between the groups and, thus, the null hypothesis was rejected. The ten M.M.P.I. scales revealed no significant differences between the groups on the individual scales. However, when examined collectively, the aftercare group scored higher than the no-aftercare group on all scales except Self-Sufficiency (which is scored in the opposite direction, corroborating the tendency in the other scales). A discriminant function correctly classified seventy-three percent of the subjects. These results indicate that the aftercare subjects probably viewed themselves as “needing” more help. The significant predictor variables found included prior familial and personal experiences similar to those bringing the subject to the Unit, employment status, age, diagnostic designation, length of hospitalization, referral planning, and self-ratings on a mood scale which was administered upon discharge from the Crisis Unit. These variables were obtained with less effort than the psychological test data. It was found that the aftercare group (compared to the other group) was younger, had a higher rate of unemployment, and had a higher rate of familial and prior personal experiences. They were also diagnosed more frequently as psychotic, with depression ranking second, and rated themselves lower on the mood scale scores. However, the difference between the before and after mood scale scores revealed that these subjects felt they had “gained” more than the no-aftercare subjects. The no-aftercare group was diagnosed more frequently as depressed, with behavior/character disorders ranking second. They tended to rate themselves higher on the mood scale scores. However, the differences between the before and after mood scale scores revealed that they had not “progressed” as much as the aftercare subjects. Although not statistically significant, it was found that the aftercare subjects were hospitalized two days longer than the subjects of the no-aftercare group. More significant is the fact that the aftercare group had a higher rate of rehospitalization than the no-aftercare group. Data collected concerning the referral process revealed that aftercare subjects were more frequently referred for treatment than were subjects of the no-aftercare group. It was speculated that those subjects who perceived themselves and/or were perceived as being "sicker" would seek further help after discharge from the Crisis Unit. The findings also suggested that not all patients need or perceived themselves as needing further help. Identifying a patient with at-risk symptoms before the first episode of schizophrenia can be invaluable to the patient, clinician, and the community because the patient is treated early in the course of the disease where treatment is highly effective. After years of research it has been confirmed that at-risk symptoms for schizophrenia generally precede the first episode of psychosis, which results in the diagnosis of schizophrenia (Correll et al., 2010). At present however, the assessment and recognition of these symptoms for schizophrenia or APS remain under debate (Carpenter, 2009). Research has shown that utilizing low dose antipsychotic medication, cognitive behavioral therapy, and having a supportive family and social environment are essential resources to recovery and reduction of trauma for a patient experiencing symptoms of APS (Lencz, Smith, Auther, Correll, & Cornblatt, 2003; McGlashan, Walsh, & Woods, 2010; Olsen & Rosenbaum, 2006; Portland Identification and Early Referral Program, 2009; S.Trevino, personal communication, November 18, 2010). At this time research shows that early detection of APS symptoms can achieve secondary and tertiary prevention, such as delaying the onset of psychosis and reducing suicide, but does not prevent schizophrenia from occurring (McGlashan, et al., 2010.) There are various ways in which counselors can receive referrals to assess a patient for APS. The patient can be referred by a family member, a health care provider, a school administrator, a court order, another clinician, or by the patient directly. A clinical assessment encounter with the patient is then needed. The encounter should use a multi-method approach of clinical interviewing and assessment tests to diagnose and help place the patient in the appropriate treatment algorithm. At first referral to a clinician, the patient may not present with a need to assess for APS. Recognizing a broad marker for instability, such as multiple AXIS I diagnoses without clear criteria, may be an effective first clue that a patient may be exhibiting APS symptoms and warrant a diagnostic assessment. One goal of this paper is to describe the current diagnostic tools for APS, especially the Structured Interview for Psychosis-Risk Syndrome, SIPS, (McGlashan, et al., 2010). When the examining physician is not satisfied that the applicant is free from these conditions, the applicant is referred to the Committee of Three, a special standing committee appointed by the Board of Social Protection for determining whether or not a license to marry should be granted. In the spring of 1970 such a situation was referred to the committee for its consideration. As a result of the committee's involvement with this applicant, the question of enforceability of the current physical mental prerequisites to marriage became a concern. The committee noted that no specific definition of the individual categories was included in the statutes that could be applied objectively to the committee in determining an applicant's fitness to marry. In an attempt to clarify these areas the committee and its parent group, the Board of Social Protection, determined to investigate marriage laws of other states seeking more valid (and/or realistic) definitions of physical and mental prerequisites to marriage. After making preliminary inquiries it was realized that a more comprehensive study and evaluation were required. A study of this nature, however, required considerable time and expense, neither of which were available under the existing program structure. It was at this point that we four students from the School of Social Work became interested in the possibility of working with the Board of Social Protection in this endeavor. Both to meet the research requirement of the School of Social Work and to provide pertinent information to the Board an informal contract was formulated between us and Dr. Edward Press, State Health Officer, who serves as Secretary of the Board of Social Protection. We were requested to research and propose an enforceable marriage law to the Board which might be presented to the Oregon State Legislature for consideration. Both Wraparound’s principles and its theory of change stress the importance of youths’ active participation throughout. However, research focusing on the experiences of youth in Wraparound indicates that they are often not particularly engaged in the process or participating actively with their teams, and the findings point to a lack of alliance between the young people and their teams. This article describes a randomized study testing the Achieve My Plan (AMP) enhancement for Wraparound, which is intended to increase young people’s satisfaction, active engagement and self-determined participation in Wraparound, as well as their alliance with the team. Study findings showed that, relative to youth who received “as usual” Wraparound, young people who received Wraparound with the AMP enhancement participated more—and in a more active and self-determined manner—with their teams. They also rated their alliance with their Wraparound teams significantly higher. Furthermore, adult team members in the intervention condition rated team meetings as being more productive, and they were more likely to say that the AMP meetings were “much better than usual” team meetings. Findings support the idea that it is possible—using a low-cost, low-“dose” intervention—to enhance young people’s self-determination and their engagement in Wraparound without detracting from team functioning or the satisfaction of other team members. The setting was the Crippled Children's Division of the University of Oregon Medical School. The purpose of the study was to test the hypothesis that one session conferencing imparting diagnostic information to parents of handicapped children reduces recall. Additionally, it was hypothesized that multi-discipline conferencing is related to greater diagnostic recall than single discipline conferencing. The sample was made up of 20 parent units whose children were evaluated at the Crippled Children's Division for suspected Cerebral Palsy and/or Mental Retardation. There were ten families from the Cerebral Palsy Clinic and ten from the Mental Retardation Clinic. Three comparisons were made: (1) The amount of information presented at the staffing session was the baseline against which the amount of information imparted at the parent conference was compared. (2) The amount of information imparted at the parent conference was compared with the amount of information that was recalled six weeks later at the follow up interview. (3) The amount of information recalled at the time of the follow up interview was compared with the staffing information in order to determine the loss of diagnostic information.   Citation × Citation Citation Abstract Copy Title: Specialized Out-of-Home Care Project: An Outcome Study Author: Arden, Carol Teresa, Braeutigam, Bonnie Jean, Schilling, Dennis, Wellman, Charlotte Mary Year: 1976 This research practicum is an outcome study of the Specialized Out-of-Home Care project (S.O.H.C.). The project, administered by the Oregon Children's Services Division, was designed to provide alternative care resources to specifically meet the needs of Portland's juvenile target offenders, who required out-of-home care. The juvenile offenders accepted into the S.O.H.C. project were between the ages of ten to eighteen, and had been adjudicated for target ctimes. The S.O.H.C. project was federally funded for a twenty-nine month period beginning on May 1, 1974 and extends through September of 1976. This study will include only the clients referred and placed during a one-year period, from July 1, 1974 to June 31, 1975. Additional "up-date" information was collected in October of 1975. A variety of alternative care resources were utilized as placements for the client population. Clients were placed in foster families, group homes, day-care settings and residential centers. This practicum was undertaken as a cooperative effort by four M.S.W. students, of Portland State University's School of Social Work, working conjointly with the S.O.H.C.'s project staff in the over-all evaluation of the project. The purpose of this study was to assess the impact that services provided by the S.O.H.C. project have had on the target population of juvenile offenders. Title: Specialized Out-of-Home Care Project: An Outcome Study Author: Arden, Carol Teresa, Braeutigam, Bonnie Jean, Schilling, Dennis, Wellman, Charlotte Mary Year: 1976 This research practicum is an outcome study of the Specialized Out-of-Home Care project (S.O.H.C.). The project, administered by the Oregon Children's Services Division, was designed to provide alternative care resources to specifically meet the needs of Portland's juvenile target offenders, who required out-of-home care. The juvenile offenders accepted into the S.O.H.C. project were between the ages of ten to eighteen, and had been adjudicated for target ctimes. The S.O.H.C. project was federally funded for a twenty-nine month period beginning on May 1, 1974 and extends through September of 1976. This study will include only the clients referred and placed during a one-year period, from July 1, 1974 to June 31, 1975. Additional "up-date" information was collected in October of 1975. A variety of alternative care resources were utilized as placements for the client population. Clients were placed in foster families, group homes, day-care settings and residential centers. We were concerned, therefore, to avoid as far as possible subjective interpretations of behavior. We decided to look for a sample of women who had already reached a measure of achievement in a competitive field traditionally dominated by men, and who were still available for questioning. Medical and rental students seemed the most appropriate choice. We wanted to know whether these women differed with regard to their sibling positions from their male colleagues on the one hand, and from the students in the School of Social Work, which has been traditionally a more female profession, on the other. Questionnaires were distributed at the local Schools of Medicine, Dentistry, and Social Work, asking students to identify their positions in their families by indicating sexes and ages of their siblings. It was found that a significantly greater number of female students are without older siblings than male students. These results appeared both in the medical-dental and in the social work sample. When older siblings were divided by sex, it was found that this relationship is stronger for opposite sex siblings than for same sex siblings. It was concluded that having older siblings is a handicap for a female wishing to go to graduate school, but not for a male, and that having older brothers is somewhat more of a handicap for her than having older sisters. This report is a result of collaboration between CEdO, Portland State University (PSU), and the Coalition of Communities of Color (CCC). Researchers from PSU conducted all of the original research. This report builds on previous work in Oregon and around the country, but it is not a duplication of existing research. Instead, the research is a novel contribution because of its extensive use of focus groups, inclusion of culturally specific focus groups, detailed thematic analysis between and among stakeholder groups, and deep-dives in the areas of students with disabilities and Native American students. The report’s literature review gives the reader a comprehensive foundation that defines the terms and measures associated with school attendance, shows the connection between attendance and academic outcomes, provides statistics related to chronic absenteeism and achievement for Oregon, provides a framework for understanding the reasons for absenteeism, and details current practices that are considered the best for schools, districts, and states to increase attendance. The review shows that although chronic absenteeism affects students of all ages, it is particularly a problem for students of color, students with disabilities, and students living in poverty. Finally, the problem is complicated, and requires a range of interventions, tailored to specific communities that address every context of students’ lives. The review demonstrates that there is still a good deal to learn about chronic absenteeism and that conducting focus group research will paint a better picture of the Oregon context and identify benefits and drawbacks of specific practices for specific locales. We are currently conducting several studies about the health care experiences and well-being of autistic adults. Lessons Learned: We have learned a number of strategies that integrate technology and process to successfully equalize power and accommodate diverse communication and collaboration needs. Conclusions: CBPR can be conducted successfully with autistic self-advocates. Our strategies may be useful to other CBPR partnerships, especially ones that cannot meet in person or that include people with diverse communication needs.
